Benefit Assistance Company, LLC will make every effort to provide competitive proposals to current and prospective clients and brokers. Our target market is employer groups ranging in size from 50 to 5,000 employees.

Listed below is the information generally needed in order to provide a credible quotation:

1. Current benefit plan design
     
2. If currently partially self funded, current specific deductible, specific and aggregate contract types
   
   
3. Two years of claims and enrollment experience, preferably by month
   
   
4. Large claim disclosure including amount of claim, diagnosis and prognosis if available
   
   
5. Current employee census
   
   
6. Name and number of years with current carrier
   
   
7. Current and renewal rates
   
   
8. Type of industry
   
   
9. Effective date
   
   
10. If multi-site, zip codes for all employees to evaluate managed care networks
